Welcome to the Valley Cities Client Portal!
Take charge of your wellness by signing up for the Client Portal through Valley Cities. When you sign up, you will have 24/7 access to your treatment plan, medications list, appointment history, lab results, and more. An additional benefit is scheduling/cancelling appointments and requesting medication refills through the Client Portal messaging feature.
Interested? Ask any member of your care team to generate a Registration Key, which you will use to set up your account. It’s that easy!
Proxy Access
Valley Cities is now offering Proxy Access for parents/legal guardians of a minor child’s Client Portal account and/or the account of a person over whom they have Power of Attorney (POA) or Legal Guardianship; clients aged 13+ can grant access to a designee of their choice. To sign up for a Proxy account, please select the applicable form from the two listed below, then return the fully completed form to Valley Cities.
